Approve retroactive Intergovernmental Agreement No. IGT-23-0027 with the California Department of Health Care Services in the amount of $574,459.20 for three years, to allow Mendocino County Public Health to participate in the Medi-Cal Managed Care Capitation Rate Increase Intergovernmental Transfer to secure additional federal revenue to deliver expanded services to the Managed Medi-Cal population, effective January 1, 2023 through June 30, 2026; authorize the Health Services Director to sign any future amendments to the Agreement that do not increase the total amount; and authorize Chair to sign the same.

Summary of Request:
The Medi-Cal program through the Department of Health Care Services (DHCS) works with local government entities and managed care Medi-Cal Plans to implement a specific type of intergovernmental transfer (or IGT) known as the “Rate Range IGT.” These IGT’s allow local government to bring additional federal dollars into the Medi-Cal system to improve health care access and outcomes for Medi-Cal beneficiaries.
Mendocino County Public Health (MCPH) is seeking approval from the Board of Supervisors (BOS) for the IGT transaction, which will be based on actual health plan enrollment figures for the services period of January 1, 2023 through December 31, 2023. Through the proposed retroactive Agreement, MCPH will transfer public health realignments funds totaling $574,459.20 ($459,567.36 estimated contribution plus a 20 percent assessment fee of $114,891.84) to DHCS, who will use the funds to draw down additional federal dollars. MCPH will receive an amount approximately equal to its own transferred funds plus a net new federal matching funds to support health services for Medi-Cal beneficiaries.
Historically, DHCS issues IGT Agreements for counties to execute retroactively. MCPH received the proposed retroactive Agreement post July 1, 2024, and therefore is requesting BOS approval of the retroactive Agreement.
