Summary of Request:
Laytonville Healthy Start (LHS) is a family resource center located in Laytonville, CA, focusing on assisting families with children in the Laytonville area. LHS was formulated under Harwood Memorial Park, Inc., a local non-profit organization, and the Laytonville School District. Mendocino County has contracted with LHS for many years for various outreach, educational, and supportive activities and services to families and children in the Laytonville area.
On March 26, 2024, the Mendocino County Purchasing Agent executed Agreement No. PA-24-58, PH-23-040 with Hardwood Memorial Park dba LHS in the amount of $28,072 for CalFresh Healthy Living Nutrition and Physical Activity Branch activities, in compliance with CalFresh Healthy Living Nutrition Education and Obesity Prevention Branch grant requirements. Services under Agreement No. PA-24-58, PH-23-040 include: approved nutrition education and cooking demonstrations; Harvest of the Month taste testing; promotion of physical activity; and brochures, posters, and social media related to exercise.
Agreement No. PA-24-58, PH-23-040 was processed by the Public Health Department (PH) with a start date of upon signing. The Mendocino County Purchasing Agent executed Agreement No. PA-24-58, PH-23-040 on March 26, 2024 with services authorized to begin on that date. PH later discovered services needed to date back to October 1, 2023, and began processing the proposed Amendment to Agreement No. PA-24-58, PH-23-040, moving the start date back from March 26, 2024 to October 1, 2023. Once the proposed retroactive Amendment is executed, Harwood Memorial Park dba LHS will provide services from October 1, 2023 through September 30, 2025.
