Summary of Request

On September 21, 2018, the Mendocino County Health and Human Services Agency (HHSA) entered into a Data Use Agreement with Partnership HealthPlan of California (PHC). The Agreement provides access to data related to improving health, case management, and care coordination for the target population of Medi-Cal beneficiaries, as clients enrolled in the Whole Person Care (WPC) pilot project, effective through December 31, 2020. This data is required in order to report important metrics, and to carry forward collaboration that was developed during the WPC pilot project, which is now the California Advancing and Innovating Medi-Cal (CalAIM) Initiative. On September 1, 2021, HHSA entered into an Amendment to the Data Use Agreement, extending the termination date through December 31, 2022.

 

The proposed second Amendment is effective as of January 1, 2023, and extends the termination date further through December 31, 2023, and updates the attached Business Associate Agreement.  This second Amendment is retroactive due to the program receiving the documents from PHC after the term start date of January 1, 2023.
