Summary of Request

WellSky Corporation (WellSky) hosts the Mendocino County Homeless Services Continuum of Care’s (MCHSCoC) Homeless Management Information System (HMIS) software. HMIS software, which is used by local homeless service providers for data collection, is required by 24 CFR Part 583, the federal regulation that establishes operational and funding requirements for the Continuum of Care program. Without an HMIS, the MCHSCoC and local homeless services providers within Mendocino County would be ineligible for mainstream federal and state homeless funding.

The proposed Agreement with WellSky secures Professional Services to configure and execute programming to export a custom file from the HMIS to be imported to Partnership HealthPlan of California (PHC) for Housing and Homelessness Incentive Program reporting. Additionally, the proposed Agreement implements Managed Services through which WellSky will provide up to 10 hours per month of consultation and training on configuration updates, system operation assessments, and system administration training. The initial term of the proposed Agreement is one-year from the effective date of the Agreement, with annual automatic renewals until terminated.
