Summary of Request:
California Advancing and Innovating Medi-Cal (CalAIM) is a new initiative by the Department of Health Care Services (DHCS) to improve the quality of life and health outcomes of Medi-Cal Members by implementing broad delivery system, program, and payment reform across Medi-Cal. The CalAIM initiative helps address the multifaceted challenges facing California's most vulnerable residents, such as individuals experiencing homelessness, children with complex care needs, justice-involved populations who have significant clinical needs, and the aging population. A key feature of CalAIM is the offering of Community Supports, which, at the option of a Medi-Cal managed care health plan and a Member, can substitute for covered Medi-Cal services as cost-effective alternatives. With the CalAIM initiative funding, the proposed Agreement will allow the Mendocino County Project Homekey program to provide Community Supports services including housing transition navigation services, housing deposits, and housing tenancy and sustaining services.
Public Health received the proposed Agreement from Partnership HealthPlan of California in November 2023, after the effective date of August 1, 2023; therefore, Public Health is now seeking execution of the retroactive Agreement.
