Approve first amendment to BOS Agreement 24-122 with Adventist Health Ukiah Valley in the amount of $100,000, for a new total of $250,000 ,to provide street medicine services to unhoused, unsheltered Medi-Cal members in the Ukiah Valley area who are enrolled in Partnership HealthPlan of California, effective upon signing through June 30, 2027; authorize the Social Services Director or designee to sign any future amendments that do not increase the maximum amount; and authorize Chair to sign same.

Summary of Request:
The Federal Government approved the State of California’s Home and Community Based Plan, to allow for a variety of programs administered by the Medicaid program, including the Housing and Homelessness Incentive Program (HHIP). HHIP is intended to improve health outcomes and access to services by addressing housing insecurity and instability as a social determinant of health for the Medi-Cal population. The goals of the HHIP program are to reduce and prevent homelessness and to ensure that Medi-Cal managed care plans, including Partnership HealthPlan of California (PHC), develop the necessary capacity and partnerships to connect members to needed housing services.
The proposed Agreement with Adventist Health Ukiah Valley is funded through Mendocino County’s allocation of HHIP funding from PHC. Through the amended Agreement, Adventist Health Ukiah Valley will comply with all HHIP rules and regulations while providing a Street Medicine Program to unhoused, unsheltered Medi-Cal members enrolled in PHC. The primary goal of this Agreement is to support interdisciplinary outreach to unhoused, unsheltered community members, with clear and direct linkages to primary care and community-based programs that can assist with housing navigation and homelessness-related services.
