Summary of Request:
Health and Human Services Agency (HHSA), Public Health, is seeking approval to apply for grant funds to provide nutrition education and obesity prevention services, “Supplemental Nutrition Assistance Program Education” (SNAP-Ed), to eligible low-income families in Mendocino County. The program is funded federally by the United States Department of Agriculture and administered at the state level by the California Department of Public Health (CDPH). Locally the grant will be administered by HHSA, Public Health, Community Health, Education & Engagement unit, to provide nutrition education and support for healthy Policy, System, and Environmental community changes. These services will target low-income individuals and families county-wide, to the extent possible. All activities will be provided by existing staff and community subcontractors.
CDPH released the attached Funding Application Packet in redline to show modifications. HHSA, Public Health, will submit the application to CDPH by March 15, 2019.
