Approve third amendment to BOS Agreement 20-108 with Eversole Mortuary extending the termination date from June 30, 2024 to June 30, 2025, and increasing the total maximum Agreement amount by $33,333.33 for a new Agreement total of $186,666.66 for the continuation of mortuary services within the Central Operations Sector of Mendocino County effective July 1, 2024, through June 30, 2025; authorize the Sheriff to sign any future amendments that do not affect the maximum Agreement total; and authorize Chair to sign same.

Previous Board/Board Committee Actions:
On July 20, 2020, the Board approved a three-year Agreement with Eversole Mortuary in the amount of $100,000.00. On January 10, 2023, the Board approved the first amendment increasing the Agreement total by $20,000 due to a surge in the number of coroner cases in Mendocino County. On June 6, 2023, the Board approved the second amendment that extended the termination date to June 30, 2024 and increased the Agreement total by $33,333.33 for a new Agreement total of $153,333.33.
Summary of Request:
One February 18, 2020, the Sheriff’s Office released Request for Proposal (RFP) SO-2020-001 for Coroner and Pathology-Related Mortuary Services. Eversole Mortuary was awarded a three-year Agreement for services performed within the Central Operations Sector of Mendocino County, with the option to extend the Agreement termination for two additional one-year periods. This third amendment will extend the termination date set out in BOS Agreement 20-108-A2 from June 30, 2024, to June 30, 2025, and will increase the Agreement total by $33,333.33 bringing the total maximum Agreement total to $186,666.66.
