Summary of Request

California received targeted expenditure authority for the “Providing Access and Transforming Health” (PATH) initiative as part of its Section 1115 demonstration renewal to scale whole-person approaches to care statewide with a clear equity lens, a strong foundation for integrated, comprehensive care, and a smooth transition from WPC and Health Homes that retains investments made by the state, local partners, and the federal government. PATH will provide funding for services to members during the transition to CalAIM and will also provide tools and resources to county and community-based providers including public hospitals, county, city, and other government agencies, justice agencies, community-based organizations (CBOs), Medi-Cal Tribal and Designees of Indian Health Programs and others to ensure a successful implementation.

 

The Capacity and Infrastructure Transition, Expansion, and Development (CITED) Initiative will provide funding to enable the transition, expansion, and development of Enhanced Care Management (ECM) and Community Supports capacity and infrastructure.

 

Public Health (PH) is seeking ratification from the Board due to the short grant submission period of 30 days. DHCS released the PATH CITED Round 3 grant application on January 15, 2024, and PH applied on February 15, 2024, with a requested funding amount of $382,597.72. If PH is awarded these funds, they will be utilized to expand program capacity by modifying and developing the necessary referral, billing, data reporting, and other infrastructure and IT systems, to support integration into CalAIM. Specifically, PH intends to implement Enhanced Care Management (ECM) across four (4) Public Health Programs, California Children’s Services(CCS), Health Care Program for Children in Foster Care (HCPCFC), California Home Visiting (CHVP), and Women, Infants, and Children (WIC) by implementing an Electronic Health Record (EHR) system to provide whole-person care management above and beyond the scope of the individual services to serve as the single point of accountability to ensure coordinated care management across multiple systems and programs.

 

 

PH also requests BOS authorization for the Director of Public Health or designee to execute the PATH CITED Round 3 funding agreement, if awarded.

 

The Notice of Award (NOA) date, and program term dates have not yet been released by DHCS.
