Summary of Request

In 2022, the Department of Health Care Services (DHCS) introduced California Advancing and Innovating Medi-Cal (CalAIM), a transformative framework designed to enhance the delivery system, program, and payment reforms across the Medi-Cal program. CalAIM leverages Medicaid as a tool to address some of California’s most pressing challenges, particularly among the state’s most vulnerable populations. This initiative targets individuals facing homelessness, those with mental health and/or substance use disorders, and people involved in the criminal justice system. Research shows that the top 5% of Medi-Cal beneficiaries-those with the highest needs-account for the majority of costs within the system. By focusing on these individuals, CalAIM aims to deliver more effective care that improves health outcomes while reducing long-term costs.

 

As part of this reform, DHCS introduced Enhanced Care Management (ECM), which focuses on addressing both clinical and non-clinical needs of high-cost, high-need beneficiaries. ECM provides a collaborative, whole-person care approach, delivering intensive and comprehensive services designed to improve health and address social determinants of health. The program works to connect vulnerable populations with necessary services through comprehensive case management.

 

Under CalAIM, California has contracted with Managed Care Plans, including Partnership HealthPlan of California (PHC), to build an ECM provider network for Medi-Cal enrollees. Mendocino County Department of Public Health is seeking the Board of Supervisors’ approval to enter into a contract with PHC to join this provider network. This partnership would allow the County to provide coordinated, interdisciplinary, community-based, high-touch, and person-centered care to specific high-need populations, ensuring that those most in need receive the comprehensive services they require to improve their health and well-being.
