Approve retroactive amendment to Agreement No. BOS 22-199 with VIVA Social Impact Partners in the amount of $7,500 for a new total of $42,575, to work in partnership with Mendocino County Public Health and the Oral Health Program Coordinator to support the Oral Health Program Objectives, and the Advisory Committee's activities, effective July 1, 2022, through a new end date of September 30, 2023 (previous end date: June 30, 2023); authorize the Chief Executive Officer or designee to sign any future amendments to the Agreement that do not increase the annual maximum amount; and authorize Chair to sign same.

Summary of Request:
The Oral Health Grant from the State of California includes funding to hire a consultant to assist the local Grant Coordinator in achieving the Oral Health Grant’s goals and objectives throughout Mendocino County. VIVA Social Impact Partners offers a unique knowledge of Mendocino County Oral Health procedural requirements and has worked in collaboration with the Mendocino County Oral Health Program since 2018.
On August 2, 2022, the Board of Supervisors (BOS) approved Agreement No. BOS-22-199 with VIVA Social Impact Partners to assist the local Grant Coordinator in achieving the State of California Oral Health Grant’s goals and objectives throughout Mendocino County, as well as developing a Community Health Improvement Plan, Action Plan, and Evaluation Plan for the program for fiscal year 2022-2023.
The proposed Amendment increases the total amount to $42,575, and extends the term end date to September 30, 2023, to allow VIVA Social Impact Partners to finish the previous fiscal year evaluation and close out the activities and goals in line with the expectations of the State of California Oral Health Grant.
This Amendment is retroactive due to the staff time required to revise the budget and payment terms in collaboration with VIVA Social Impact Partners.
