Previous Board/Board Committee Actions:
On June 7, 2016 the Board of Supervisors was notified of mold intrusion in the Mendocino County Museum. On July 12, 2016 the Board approved agreement 16-051.
Summary of Request:
On May 23, 2016, the Risk Management Division of the Executive Office was notified of significant mold damage in various areas of the Mendocino County Museum and retained an industrial hygienist to test the facility to determine the scope and extent of the issue. On June 7, 2016 testing conducted during the investigation confirmed the presence of sporadic mold intrusion in some portions of the Museum and it was closed as a precautionary measure. After further testing, some portions of the Museum were reopened to the public on June 10, 2016. Repairs on this project took an extensive length of time, and were not officially completed until July 29, 2017 with the final completion of the Artifacts Storage roof. Upon final audit of the public project records, and in consultation with the Executive Office staff, it was discovered that although the change order(s) were signed and approved by the ACEO/Risk Manager, Board approval is required via a contract amendment pursuant to County Policy and Public Contract Code Section 22050. The Executive Office requests that this amendment be retroactively approved in order to allow final payment and subsequent reimbursement for the final loss.
