Summary of Request

The California Department of Social Services (CDSS) is seeking proposals from counties interested in implementing a new California Work Opportunity and Responsibility to Kids (CalWORKs) Home Visiting Initiative (HVI). The purpose of HVI, which will be implemented jointly by the Public Health and Social Services Branches of the Health and Human Services Agency, is to support positive health, development, and well-being outcomes for pregnant and parenting women, families, and infants born into poverty to improve the likelihood that they will exit poverty.

 

If Mendocino County is awarded implementation funds, local CalWORKs participants electing to participate in the HVI will receive regular, planned home visits from a trained professional who will provide guidance, coaching, access to prenatal and postnatal care, and other health and social services. Other components of HVI include the development of a collaborative case management plan that builds on strengths established during home visits, close coordination and communication with CalWORKs staff, and encouragement for families to enroll their children in high-quality early learning settings.

 

To apply, Mendocino County must submit a Request for County Plan to CDSS by September 28, 2018. The Request will outline the evidence-based home visiting model (the pre-approved Healthy Families America model) currently used by the Healthy Families Home Visiting program of the Health and Human Services Agency, Public Health Branch. If approved, the County will receive a minimum of $57,537 per 6-month cycle over the course of the initial 18-month application period of January 1, 2019 to June 30, 2020. Re-application will be required for subsequent periods if funding is appropriated.
