Summary of Request:
On March 11, 2022, the Mendocino County Purchasing Agent (PA) approved Agreement No. PA 22-49, PH-21-022 with DG Creative Branding, Inc., to provide professional services to Mendocino County Public Health (PH) including: collaborating to create effective, attractive, and professional branding solutions, acting as a subject matter expert and strategy consultant, and drafting and designing materials intended for print or online use. Additionally, DG Creative Branding, Inc., was contracted to facilitate the creation of an independent website and InterestNET for PH.
In June 2023, the department determined there was a need to increase the amount of Agreement No. PA 22-49, PH-21-022 in order to execute final payments to DG Creative Branding, Inc. The proposed Amendment to Agreement No. PA 22-49, PH-21-022 increases the total amount from $50,000 to $55,500. Once the proposed Amendment is executed, it will be effective for the full term of the Agreement, from March 11, 2022 through July 30, 2023.
Due to an unexpected need to increase the total contract amount, this Amendment was not anticipated, therefore is retroactive.
