Approve Agreement (first amendment to Agreement No. PA 20-133) with Chapel by the Sea Mortuary in the amount of $16,666.66 for a new Agreement total of $66,666.66 to provide mortuary services within the Coastal Operations Sector of Mendocino County effective July 1, 2023 through June 30, 2024; authorize the Sheriff to sign any future amendments that do not affect the maximum agreement total; and authorize Chair to sign same.

Summary of Request:
On June 10, 2020, the Sheriff’s Office entered into Agreement No. PA 20-133 with Chapel by the Sea Mortuary in the amount of $50,000 for mortuary services throughout the Coastal Operations Sector of Mendocino County. The agreement results from RFP SO-2020-001 that was released on February 18, 2020 and has the option for two one-year extensions.
This first amendment extends the termination date set out in the original Agreement No. PA 20-133 from June 30, 2023 to June 30, 2024 and will increase the agreement total cost by $16,666.66 for the one-year term of this amendment.
