Approve Agreement with California Department of Public Health in the amount of $0 to ensure security and privacy of confidential public health records in the California Enhanced HIV/AIDS Case Reporting System, effective upon signing for a period of five years; authorize the Health Services Director or designee to sign any future amendments that do not increase the Agreement amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
The California Department of Public Health (CDPH) releases a California Enhanced HIV/AIDS Case Reporting System Data Use and Disclosure Agreement every five years to all County Public Health Departments to ensure the security and privacy of all HIV/AIDS Case Reporting System data, and other personal and confidential information, disclosed to data recipient by CDPH. Mendocino County Public Health is obligated to enter into this proposed Agreement to ensure the continued transmission of personally Identifiable Information between our county and CDPH.
