Approve amendment to Agreement No. PH-24-020 with Angela Fischer-Weaver, PA-C, in the amount of $40,100 for a new agreement total of $65,000 to provide Physician Assistant Certified services at the Mendocino County Juvenile Hall, effective August 14, 2024, through June 30, 2025; and authorized the Health Services Director to sign any future amendments that do not increase the maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
Angela Fisher-Weaver provides an important role in the Mendocino County Juvenile Hall medical program by providing Physician Assistant Certified (PA-C) services that consist of but not limited to, medical services, prescription writing, and referrals.

The proposed Amendment will allow Angela Fisher-Weaver, to continue to provide these crucial PA-C services needed for the Mendocino County Juvenile Hall clientele. The original contract amount was $24,900. This amendment proposes an increase of $40,100 for a new total of $65,000. Once the proposed Amendment has been approved it will be effective through June 30, 2025.
