Approve retroactive second Amendment to the Data Use Agreement with Partnership HealthPlan of California, as a no-cost Agreement for sharing data on clients in common, for the purpose of Care Coordination reporting and Case Management, effective January 1, 2023 through December 31, 2023 (original end date: December 31, 2022); authorize the CEO or designee to sign amendments that do not increase the maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
On September 21, 2018, the Mendocino County Health and Human Services Agency (HHSA) entered into a Data Use Agreement with Partnership HealthPlan of California (PHC). The Agreement provides access to data related to improving health, case management, and care coordination for the target population of Medi-Cal beneficiaries, as clients enrolled in the Whole Person Care (WPC) pilot project, effective through December 31, 2020. This data is required in order to report important metrics, and to carry forward collaboration that was developed during the WPC pilot project, which is now the California Advancing and?
