Ratify submission of application and approve revenue Agreement with Partnership HealthPlan of California, for Incentive Payment Program funding in the amount of $207,000, for the implementation of California Advancing and Innovating Medical projects, effective upon signing through December 31, 2025; authorize the Social Services Director or designee to sign the Revenue Agreement and any future amendments that do not increase the maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
On February 27, 2024, the County of Mendocino, Board of Supervisors (BOS) approved a revenue Agreement with Partnership HealthPlan of California (PHC) to provide Community Supports Services using California Advancing and Innovating Medi-Cal (CalAIM) funds. Services under the Agreement began on August 1, 2023, including housing, transition navigation services, housing deposits, and housing tenancy and sustaining services.

In April 2024, the Department of Social Services (SS), Special Projects Team applied for related funding from PHC for additional CalAIM funds for an Incentive Payment Program (IPP) to assist in the development of the Community Supports Services...
