SUMMARY OF REQUEST:

Mendocino County Department of Social Services has entered into a revenue agreement with Partnership HealthPlan, through an MOU process, to provide Community Supports and Enhanced Care Management services to eligible Medi-Cal beneficiaries that we serve. Initially, these services will be limited to Housing Navigation and Housing Tenancy Supports provided by staff in the Special Projects Team in DSS. Over time, DSS leadership may choose to expand services to additional eligible populations, including families engaged in child welfare services and/or seniors at risk of placement in long term care.

The Social Services Department has never received per-member-per-month reimbursement from a Medi-Cal Managed Care plan for services provided.
