Approve Agreement with Partnership HealthPlan of California, a per member per month fee for service revenue Agreement, for the Enhanced Care Management (ECM) Program, effective upon signature, with automatic yearly renewals until either party terminates the Agreement; authorize the Health Services Agency Director or designee to sign Agreement and any renewals or amendments that do not exceed the annual maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
In 2022, the Department of Health Care Services (DHCS) introduced California Advancing and Innovating Medi-Cal (CalAIM), a transformative framework designed to enhance the delivery system, program, and payment reforms across the Medi-Cal program. CalAIM leverages Medicaid as a tool to address some of California's most pressing challenges, particularly among the state's most vulnerable populations. This initiative targets individuals facing homelessness, those with mental health and/or substance use disorders, and people involved in the criminal justice system. Research shows that the top 5% of Medi-Cal beneficiaries-those with the highest needs-account for the majority of costs within the system. By focusing on these individuals, CalAIM aims to deliver more effective care that improves health outcomes while reducing long-term costs.
