Approve retroactive Agreement with Applied Survey Research in the amount of $49,900, to conduct the 2024 Homeless Point-In-Time Census and Survey Project in Mendocino County, using Housing and Homelessness Incentive Program allocation funds, effective January 1, 2024, through December 31, 2024; authorize the Social Services Director or designee to sign any future amendments that do not increase the maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
The Federal Government approved the State of California's Home and Community Based Plan, to allow for a variety of programs administered by the Medicaid program, including the Housing and Homelessness Incentive Program (HHIP). HHIP is intended to improve health outcomes and access to services by addressing housing insecurity and instability as a social determinant of health for the Medi-Cal population. The goals of the HHIP program are to reduce and prevent homelessness, and to ensure that Medi-Cal managed care plans, including Partnership HealthPlan of California (PHC), develop the necessary capacity and partnerships to connect members to needed housing services.
