Approve second amendment to BOS Agreement No. 20-108 with Eversole Mortuary in the amount of $33,333.33 for a new Agreement total of $153,333.33 to provide mortuary services within the Central Operations Sector of Mendocino County effective July 1, 2023 through June 30, 2024; authorize the Sheriff to sign any future amendments that do not affect the maximum agreement total; and authorize Chair to sign same.

PREVIOUS BOARD/BOARD COMMITTEE ACTIONS:
On July 20, 2020, the Board approved a three-year agreement with Eversole Mortuary with the option for two one-year extensions. On January 10, 2023, the Board approved the first amendment with Eversole Mortuary increasing the agreement total by $20,000 due to a surge in the number of Coroner cases in Mendocino County.
SUMMARY OF REQUEST:
Agreement No. BOS 20-108 is a three-year agreement that expires on June 30, 2023 and has the option for two one-year extensions. The agreement results from RFP No. SO-2020-001 that was released on February 18, 2020.
This second amendment extends the termination date set out in the original Agreement No. BOS 20-108 from June 30, 2023 to June 30, 2024 and will increase the total cost by $33,333.33 for the one-year term of this amendment.
