Approve retroactive Agreement (First Amendment to Agreement No. PA 22-49, PH-21-022) with DG Creative Branding, Inc., in the amount of $5,500 for a new total of $55,500, to provide professional branding solutions to Mendocino County Public Health, effective March 11, 2022 through July 30, 2023; authorize the Chief Executive Officer or designee to sign any future Amendments to the Agreement that do not increase the annual maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
On March 11, 2022, the Mendocino County Purchasing Agent (PA) approved Agreement No. PA 22-49, PH-21-022 with DG Creative Branding, Inc., to provide professional services to Mendocino County Public Health (PH) including: collaborating to create effective, attractive, and professional branding solutions, acting as a subject matter expert and strategy consultant, and drafting and designing materials intended for print or online use. Additionally, DG Creative Branding, Inc., was contracted to facilitate the creation of an independent website and InterestNET for PH.
