SUMMARY OF REQUEST:
The National Association of County and City Health Officials has funding available for local health departments to engage in activities that will demonstrate a measurable increase in their readiness to achieve Public Health Accreditation Board (PHAB) accreditation. Mendocino County HHSA has commenced its accreditation plans which are now in full swing. Grant activities will focus on developing a Community Health Improvement Plan, which lays the groundwork for community programs, policies and interventions with Mendocino County Local Public Health System. There are no cost-sharing or matching requirements. If granted, contract term will be six months (FFY 2016/17) with funds ranging up to $15,000.
