Approve fourth amendment to Standard Agreement 14-10518 with California Department of Public in the amount of $7,396, for a new total of $1,122,642, to provide emergency and disaster preparedness planning for medical and health events in FY 2014-18; authorize the Health and Human Services Agency Director or designee to sign any future amendments to the Agreement that do not affect the annual maximum amount; and authorize Chair to sign same.

Summary of Request

This Agreement with CA Dept. of Public Health and Mendocino County HHSA PH provides funding for staffing, equipment, training, and supplies to Public Health, clinic, hospital, and Skilled Nursing Facilities (SNF) partners for disaster planning and response to health and medical events in Mendocino County. Contract amount has been increased to restore decrease from previous Amendment.
