Summary of Request:
Whole Person Care (WPC) is a program within California’s Section 1115(a) Medicaid Waiver Renewal, entitled Medi-Cal 2020, which was approved by the Centers for Medicare and Medicaid Services (CMS) on December 30, 2015. WPC Pilots were designed to provide support to integrate care for a particularly vulnerable group of Medi-Cal beneficiaries who have been identified as high users of multiple systems and continue to have poor health outcomes. WPC Pilots focus on the coordination of health, behavioral health, and social services, as applicable, in a patient-centered manner with the goals of improved beneficiary health and well-being through more efficient and effective use of resources.
Mendocino County Health and Human Services Agency (HHSA) has worked with community partners to identify its target population for the WPC Pilot to include Medi-Cal beneficiaries who have serious mental illness (SMI), prioritizing those who are high utilizers of mental health and/or medical services and those with the following additional barriers: homelessness or housing instability, co-occurring Substance Use Disorder (SUD), and/or recent interactions with the criminal justice system. HHSA’s goal is care coordination to divert high utilizers away from Emergency Room and inpatient services towards care that addresses the underlying behavioral health and socioeconomic needs to improve health outcomes and reduce utilization of unnecessary services.
