PREVIOUS BOARD/BOARD COMMITTEE ACTIONS:
On June 20, 2017, the Board of Supervisors adopted resolution 17-078 ratifying the application submission for Whole Person Care Pilot Program, Second Round, with the California Department of Health Care Services. Upon execution of the subsequent agreement, BOS# 17-042, the following subcontracts were approved by the Board: Redwood Quality Management Company, $120,000, BOS# 17-156; Mendocino Community Health Clinic, $60,000, BOS# 17-144; and Mendocino Coast Clinics, $60,000, BOS# 17-153. On January 2, 2018, this item was referred by the Board to the Public Health, Safety, and Resources Committee.

SUMMARY OF REFERRAL:
Whole Person Care (WPC) is a program within California's Section 1115(a) Medicaid Waiver Renewal, entitled Medi-Cal 2020, which was approved by the Centers for Medicare and Medicaid Services (CMS) on December 30, 2015. WPC Pilots were designed to provide support to integrate care for a particularly vulnerable group of Medi-Cal beneficiaries who have been identified as high users of multiple systems and continue to have poor health outcomes. WPC Pilots focus on the coordination of health, behavioral health, and social services, as applicable, in a patient-centered manner with the goals of improved beneficiary health and well-being through more efficient and effective use of resources.
