Approve fifth amendment to BOS Agreement number 15-098 (State Revenue Agreement No. 15-10071) with the California Department of Public Health in the amount of $0, for an Agreement total of $$4,296,562, to provide funding for the Women, Infants and Children Program of the Health and Human Services Agency, Public Health Branch, effective when Agreement becomes fully executed through September 30, 2019; authorize the Health and Human Services Agency Director or designee to sign any future amendments that do not exceed the annual maximum amount; and authorize Chair to sign same.

Summary of Request:
The California Department of Public Health (CDPH) provides funds annually to the Women, Infants and Children (WIC) Program of the Health and Human Services Agency, Public Health Branch, to support pregnant, breastfeeding, or postpartum women and their children. These funds enable the WIC program to provide services to help mothers and young children stay healthy. Specifically, qualifying WIC participants are eligible to receive nutrition education, vouchers for nutritious food, breastfeeding support, and referrals to medical providers and community services. WIC provides these supportive services at nine locations throughout the County.
CDPH designated $3,812,782 in the original revenue Agreement to support Mendocino County’s WIC program from October 1, 2015 through September 30, 2019. Ensuing amendments increased that amount to $4,296,562 with the goal of increasing WIC participation, improving child and maternal health, and to add funding for staffing, equipment, trainings, and supplies for the WIC program.
Health and Human Services Agency is presenting this $0 amendment to the Board of Supervisors because CDPH requires Board authorization to execute the amendment.
