Approve retroactive Agreement with Ford Street Project in the amount of $25,944 to provide short term residential treatment for Finding Home participants with substance abuse disorders for the period of September 30, 2018 through September 29, 2020; authorize the Health and Human Services Agency Director or designee to sign any future amendments that do not exceed the annual maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
In 2017, the Health and Human Services Agency (HHSA) applied for and received Substance Abuse and Mental Health Services Administration (SAMHSA) funding for the Finding Home project. This project's goal is to decrease homelessness and improve the health, well-being and self-sufficiency of homeless people with co-occurring disorders (mental illness and substance use disorders).

Using SAMHSA funds, Ford Street Project will provide services for members of the homeless population in Mendocino County.
