Summary of Request:
In 2017, the Health and Human Services Agency (HHSA) applied for and received Substance Abuse and Mental Health Services Administration (SAMHSA) funding for the Finding Home project. This project's goal is to decrease homelessness and improve the health, well-being and self-sufficiency of homeless people with co-occurring disorders (mental illness and substance use disorders).
Using SAMHSA funds, Ford Street Project will provide services for members of the homeless population in Mendocino County. Such services include: care coordination with integrated Mental Health and Substance Use Disorders Treatment Services with county and community partners; appropriate, evidence-based residential and rehabilitation services for homeless individuals struggling with substance use disorders; referrals and linkage to care management and treatment services; intensive care management including access to primary health care and wrap-around support; and access to permanent housing.
The original Agreement with Ford Street Project that utilizes SAMHSA funds has a term of April 1, 2018 through September 30, 2019 for year 1 and year 2 services in the amount of $42,504. Approval of rollover SAMHSA funding, and an award letter for additional funding for a third year of services in Mendocino County, was received in July of 2019. Upon receipt of the award letter, HHSA reviewed all contracts that utilize SAMHSA funds, and created this Agreement for the third year of services provided from October 1, 2019 through September 29, 2020 in the amount of $25,944. The review was extensive, taking months of staff time, in order to ensure compliance with new grant requirements, and reduce audit risk. HHSA requests retroactive approval of this Agreement due to the retroactive nature of the SAMHSA budget rollover, and the in-depth review and adjustment process required to draft the new Agreement.
