RECOMMENDED ACTION/MOTION:

Approve retroactive Fifth Amendment to Agreement No. PH-19-079, PA No. 20-124, with Ukiah Brewing Company in the amount of $269,000 for a new total of $1,078,500 to provide meals to homebound seniors in the Ukiah and Willits area due to the COVID-19 pandemic through the Great Plates Delivered program from May 12, 2020 through December 31, 2020; delegate authority to the Purchasing Agent to sign amendments to the Agreement, including for a new total amount, that do not exceed the current invoiced amount plus the anticipated amount needed for continuation of the program at current levels (i.e., that do not increase the cost per meal, that maintain a weekly average of $60,984, and that do not exceed a projected maximum of $2,304,474 through March 31, 2021), provided that said amendments and excesses of the weekly average are reported to the Board of Supervisors; and authorize Chair to sign same.
