Approve retroactive Agreement with Mendocino Coast Hospitality Center in the amount of $49,866 to provide intensive care management and coordinated development of integrated individual service plans to support the Finding Home grant, effective September 30, 2020 through September 29, 2021; authorize the Health and Human Services Agency Assistant Director to sign any future amendments that do not exceed the annual maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
In 2017, the Health and Human Services Agency (HHSA) applied for and received Substance and Mental Health Services Administration (SAMHSA) funding for the Finding Home project. The project's goal is to decrease homelessness and improve the health, well-being and self-sufficiency of persons who are experiencing homelessness and have co-occurring disorders (mental illness and substance use disorders).

Using SAMHSA funds, Mendocino Coast Hospitality Center (MCHC) provides services for members of the homeless population in Mendocino County.
