Approve Amendment to BOS Agreement No. 17-042 with the California Department of Health Care Services in the amount of $2,286,613.08 for a new total of $12,791,333.08 for the extension of the Whole Person Care Pilot Project, effective upon full execution through a new end date of June 30, 2022 (original end date of June 30, 2021); authorize the Health and Human Services Agency Assistant Director/ Public Health Director to sign any amendments to the Agreement that do not increase the maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
Whole Person Care (WPC) is a program within California's Section 1115(a) Medicaid Waiver Renewal, entitled Medi-Cal 2020, which was approved by the Centers for Medicare and Medicaid Services (CMS) on December 30, 2015. WPC Pilots were designed to provide support to integrate care for a particularly vulnerable group of Medi-Cal beneficiaries who have been identified as high users of multiple systems and continue to have poor health outcomes.
