Approve Agreement with Ukiah Senior Center in the amount of $51,050 to provide health and welfare outreach services, financial services, and information referrals to elderly individuals and persons with disabilities in Ukiah and surrounding areas, effective July 1, 2021 through June 30, 2022; authorize the Health and Human Services Agency Assistant Director/Social Services Director to sign any future amendments that do not increase the annual maximum amount; and authorize Chair to sign the same.

SUMMARY OF REQUEST:
The proposed Ukiah Senior Center contract is for the provision of health and welfare information, referral support, and financial management for elderly individuals and persons with disabilities in the City of Ukiah and its surrounding areas. These funds ensure that the senior center maintains an outreach staff person to carry out the provisions of the contract. This includes fostering community awareness of the Ukiah Senior Center and its resources, working with Adult Protective Services (APS) staff on individual case plans, as well as assisting clients with transportation as necessary.
