Approve amendment to BOS Agreement No. 20-054 with Pacific Redwood Medical Group in the amount of $172,942 for a new agreement total of $350,254 to include LVN services to the medical services provided at Juvenile Hall, effective upon full execution through original agreement end date of June 30, 2023; authorize the Chief Probation Officer to sign any amendments to the Agreement that do not increase the maximum amount; and authorize Chair to sign same.

Board of Supervisors approved agreement with Pacific Redwood Medical Group for the period of July 1, 2020 through June 30, 2023 on May 20, 2020; Board of Supervisors approved agreement with Pacific Redwood Medical Group for the period of July 1, 2017 through June 30, 2020 on October 18, 2017.                     

 

Summary of Request

Pacific Redwood Medical Group (PRMG) provides medical services to youth detained in Juvenile Hall.  PRMG is currently contracted to provide up to 26 mid-level provider (Physician’s Assistant or Nurse Practitioner) hours per month, up to six days per week.  PRMG conducts physical examinations, episodic health care or medical consultation, referral to specialty medical services, as necessary, supervision and maintenance of medical records.  Contracted services also included working closely with our staff LVN to provide the appropriate medical care our residents require.  Additionally, PRMG is contracted to provide coverage for our LVN during unplanned periods of absence to ensure medical care is provided as required by state law at an additional hourly cost with a maximum payable amount for the life of the agreement.

 

In November 2020, Juvenile Hall’s staff LVN left employment with the County.  PRMG has provided limited LVN services to Juvenile Hall during this time, as they have provided during previous periods of absences of our LVN.  The amendment request of including the LVN services along with existing PRMG medical services will improve the coordination and provision of medical services to youth as well as simplify the coordination of care with the contracted juvenile psychiatric provider.  Additionally, PRMG will provide LVN services 5 hours per day 7 days per week, after PRMG staffing is completed, which increases services to youth to a daily basis versus the previous 4-5 days per week county staff were providing.  We further anticipate a cost savings of approximately 10-14% per year by consolidating LVN services with existing medical services provided by PRMG.  PRMG will also be responsible for all supervision of LVN personnel ensuring that all State requirements for supervision are met and complied with.
