Approve Second Amendment No. 17-10330-A02 to grant Agreement No. 17-10330 with the California Department of Public Health in the amount of $57,166 for a new total of $308,790, to provide immunization services in Mendocino County, effective upon full execution through June 30, 2022; adopt resolution approving the CDPH revenue Amendment No. 17-10330-A02; authorize the Health and Human Services Agency Assistant Director/Behavioral Health Director or designee to sign any future amendments that do not increase the maximum amount; and authorize Chair to sign same.
