Summary of Request

On June 20, 2017, the Mendocino County Board of Supervisors approved Mendocino County Health and Human Services Agency’s (HHSA) application for the Whole Person Care (WPC) Pilot Project, and executed the resulting revenue agreement with the California Department of Health Care Services (DHCS). WPC funding from the original agreement with DHCS covered five fiscal years, 2016-17 through 2020-21; with funding ending in December 2020 under the State’s Medi-Cal Waiver. In May DHCS executed an amendment for WPC services to increase the amount to $12,791,333.08 and extend the end date to June 30, 2022. WPC funding is for prioritization of services for persons with Serious Mental Illness (SMI) and additional barriers including homelessness or housing instability. The goal of the WPC program is to provide comprehensive coordinated care resulting in better health outcomes among participants with SMI.

 

Success of the WPC Pilot Program depends on the collaborative participation of community organizations. HHSA identified several community partners for the WPC Pilot Project during the WPC grant application process including Redwood Community Services, Inc. (RCS), and their participation in the WPC Pilot Project is fundamental in achieving coordinated care grant objectives. The goal of the WPC program is to provide comprehensive coordinated care resulting in better health outcomes among participants with Severe Mental Illness. Under the guidelines specified in the original agreement, RCS will participate in all relevant activities related to the continued implementation of the WPC Pilot Program including: providing work space for Wellness Coaches who will support participants in accessing a wide spectrum of medical, behavioral, and social service needs, including family finding as appropriate; attending WPC Steering Committee meetings; ensuring communication between the Behavioral Health Care Manager and Wellness Coaches; ensuring that the Behavioral Health Care Manager participates in quarterly WPC case conferences; and providing winter shelter services to WPC enrollees.  RCS will also enter relevant data into an electronic health record system to be maintained by Redwood Quality Management Company (RQMC). RQMC is required to report that data to HHSA in a separate report.

 

 The proposed amendment is in response to the amendment from DHCS to extend the contract term and increase funds for services provided.
