Summary of Request:
In 2017, the Health and Human Services Agency applied for and received Substance Abuse and Mental Health Services Administration (SAMHSA) funding for the Finding Home Project. The Project’s goal is to decrease homelessness and improve the health, well-being and self-sufficiency of persons who are experiencing homelessness and have co-occurring disorders (mental illness and substance use disorders).
Using SAMHSA Finding Home Grant funds, Redwood Community Services (RCS) provides services for individuals experiencing homelessness in Mendocino County, and supports recovery through integrated Mental Health and Substance Use Disorder treatment. Such services include: intensive care management and coordinated development of integrated individual service plans; integrated Mental Health and Substance Use Disorder treatment; intensive care management, including access to primary health care and wrap around support; and access to permanent housing. RCS provides the aforementioned services for transitional age youth, age 15-24 at the Arbor facility, and adult services are provided at Building Bridges, the Homeless/Winter Shelter. This contract is for the 5th year of a 5 year grant.
