Approve Agreement with Ukiah Senior Center, Inc., in the amount of $57,300 to provide senior health and welfare outreach, information and referral, and financial services for Adult Protective Services referrals in Fiscal Year 2016-17; authorize the Health and Human Services Agency Director to sign any future amendments to the Agreement that do not affect the annual maximum amount; and authorize Chair to sign same.

Previous Board/Board Committee Actions:
July 26, 2005, Item 5(19) approved the contract for services through the Ukiah Senior Center for Fiscal Year 2005-06; July 18, 2006, Item 5(37) approved the contract for services through the Ukiah Senior Center for Fiscal Year 2006-07; June 19, 2007, Item 5a(24) approved the contract for services through the Ukiah Senior Center for Fiscal Year 2007-08. Contracts for services continued for Fiscal Years 2008 through 2014 were below $50,000 and did not require Board of Supervisors action. August 4, 2015, Item 4(h) approved the contract for services through the Ukiah Senior Center for Fiscal Year 2015-16.
Summary of Request:
Mendocino County Health and Human Services Agency Adult Services contracts with Senior Centers throughout Mendocino County to provide outreach services to the elderly and dependent adult population. The Senior Centers serve as a resource for the elderly and are a key factor for them in maintaining their safety and well-being.
Ukiah Senior Center will provide senior health and welfare outreach, information and referral, and financial services which include: In-Home Support Services, Health Care Providers, Social Security Administration, Adult Protective Services (APS) Outreach, Energy Assistance, Senior Center Services, Community Services, Financial Services, Medical Services for Nutritional Assessments, Depression or Suicide Assessments and Drug and Alcohol Assessments.
