SUMMARY OF REQUEST:
Health and Human Services Agency (HHSA) Behavioral Health and Recovery Services (BHRS) is required to assist its clients in the least restrictive setting. Some clients are in need of residential care. The Willow Glen Care Center offers 24-hour, long-term residential care for the elderly and other adults with chronic mental health conditions. The desired outcome for residents and their families is a successful stable placement in a transitional or long-term care program that provides a safe environment, specialty mental health services, self-respect and personal dignity. This Agreement provides an opportunity to move our conserved clients who are currently placed in locked facilities to a less restrictive, therapeutic environment that includes several separate programs targeted to different needs.
