Summary of Request

With this Agreement, the Mendocino County Health and Human Services Agency aims to improve the percentage of community health center patients who successfully complete the renewal process for Medi-Cal coverage. The project is intended to reduce attrition rates for our Medi-Cal population, thereby ensuring timely access to primary health care needs and thus improve health. The Department of Health Care Services, Medi-Cal Eligibility Division, has provided this funding to counties through the implementation of SB18 to provide Medi-Cal outreach and renewal assistance services to our County’s residents. The Agreement period is July 1, 2016 through June 30, 2018.
