Ratify and approve the submission of a funding proposal to County Medical Services Program (CMSP) for a Wellness and Prevention Pilot Project to provide local-level wellness and prevention services to CMSP eligible persons in the total amount of $225,000 during fiscal years 2016-19 and authorize the Health and Human Services Agency Director to sign subsequent agreement if funding is granted.

Summary of Request

The CMSP Governing Board seeks to test the effectiveness of providing local-level wellness and prevention services to CMSP eligible persons. Projects may address any or all of the three project areas: 1) Community Wellness; 2) Whole Person Care; 3) Addressing the Social Determinants of Health over a 3-year period.  Our proposal addresses Social Determinants of Health identified in the Community Health Improvement Plan (CHIP) with outcomes to include: an increase in enrollment; an increase in enrollee participation in the primary care benefit; a decrease in mental health crises addressed in hospital emergency rooms by 5%; a decrease in 5150 arrests by 5%. 

 

This 3-year project will be accomplished in collaboration with the following Community Based Organizations (CBO): North Coast Opportunities, FIRST 5 and the Alliance for Rural Community Health. CBO expenses will pay paid by the CMSP Grant funds for Year 1 $74,841.66; Year 2, $74,010.64; and   Year 3, $72,599.12.

 

The county’s costs, to be paid with Realignment funds, are as follows: Year 1 totals $9,234.44; Year 2, $5,110.80; and, Year 3, $2,285.06 for a total of $16,630.30.  These funds will be used for existing staff time, office expenses, travel and administrative overhead. The County Match with the in-kind from other non-County resources totals $238,081.72 over the three year period.
