Summary of Request

The Community Health Improvement Project (CHIP) began on June 8, 2016 when a coalition of organizations hosted a Community Health Improvement Planning Workshop. This workshop was the first initiative of a long-term systematic effort to address public health problems in our community through collective action and development of a Community Health Improvement Plan (CHIP). 

 

The project and plan are under the direction of Healthy Mendocino Steering Committee including the Mendocino County Health and Human Services Agency (HHSA), Ukiah Valley Medical Center, Frank R. Howard Memorial Hospital, the Alliance for Rural Community Health, and North Coast Opportunities.  The CHIP plan is based on the data results of the extensive Community Health Needs Assessment activities. The CHIP plan focuses on five priorities for improving health in Mendocino County: poverty; housing; mental health; family wellness and childhood obesity; and childhood trauma. The plan dovetails with the HHSA Public Health Accreditation process.

 

CHIP Planning Group members will provide a brief synopsis of the Community Health Improvement Planning Workshop, inspirational stories from community collaboration events and the roles of all community partners in creating a healthier county (along with HHSA Director, Tammy Moss Chandler and HHSA Public Health Quality Improvement and Accreditation Program Administrator, Samantha Kinney).
