Ongoing program since 1974.

SUMMARY OF REQUEST:
The CHDP Program is a federally mandated preventive health program that serves low to moderate income children and youth. Through the CHDP program, children can obtain regular preventive health assessments to identify any health problems. Those with suspected problems are referred for necessary diagnosis and treatment and receive care coordination services through the CHDP program. The CCS Program is a federally mandated health program that provides diagnostic and treatment services, medical case management, and physical/occupational therapy services to children under age 21 with CCS-eligible medical conditions. The annual certification statements certify that both programs will comply with all applicable laws, rules and regulations; will comply with the CMS Plan and Fiscal Guidelines Manual; and the County agrees that the program(s) may be subject to all sanctions or other remedies applicable if the program(s) violate any of the laws, regulations, and policies with which it has certified it will comply.
