SUMMARY OF REQUEST:
Wraparound services are intensive home and community-based services designed to prevent high needs youth from needing out-of-home placements and/or to support them in lower levels of care after transitioning out of group home placements. The program operates at the county level, and referrals of eligible youth are initiated by Family and Children's Services (FCS), State Adoptions, and Juvenile Probation.

At times, FCS places high needs youth who are dependents of the Mendocino County Juvenile Court with family members, non-relative extended family members, or non-related foster parents living in Lake County. This Agreement funds Wraparound services for these clients in order to ensure the stability and success of their placement. This agreement is not applicable for any children/youth in the RCS Residentially Based Services (RBS) program.
