On June 26, 2016, the Board approved an amendment to Agreement No. PA 22-15 with Bio IQ.
Summary of Request:
The employee work-site wellness program contracts with Bio IQ for biometric screening and flu shot services for County employees and their eligible dependents. Agreement No. PA 18-10 for the biometric screening services does not include flu shot services and fees. Adding flu shot services increases the total agreement amount and requires Board approval.
Human Resources is requesting approval of an amendment that adds flu shot services to the agreement and increases the agreement amount by $10,854.55 for a new total agreement amount of $56,554.55 in Fiscal year 2017-18. Staff is also requesting Board authorization for the Human Resources Director to approve and sign future amendments to the agreement that do not increase the total agreement amount.
