Summary of Request

Healthy and Human Services Agency (HHSA) Housing Options for Mendocino (HOMe Team) applied for funding made available by the Substance Abuse and Mental Health Services Administration (SAMHSA) to assist communities in decreasing homelessness and improving the health, well-being, and self-sufficiency of homeless people who have co-occurring disorders (both a mental health and a substance abuse problem).

The grant was awarded to expand current activities and introduce innovative strategies such as treatment, integrated care management, wraparound support, housing access, and infrastructure improvements. By strengthening our county-wide community supports for homeless families and individuals, more will access and maintain stable housing. The contract term will be five years (FFY 2017-22) with funds of $400,000 per year.
