Approve retroactive Agreement with Mendocino Community Health Clinic, Inc., in the amount of $60,000 to provide support, implementation, and participant-specific data to the County of Mendocino's Whole Person Care Pilot Project for the term of July 1, 2017, through December 31, 2017; authorize the Health and Human Services Agency Director or designee to sign any future amendments to the Agreement that do not increase the annual maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
The Whole Person Care (WPC) Pilot Program is a significant, new, grant-funded project with substantial requirements for community participation. The goal of the WPC program is to provide comprehensive coordinated care resulting in better health outcomes among participants with Severe Mental Illness. Mendocino Community Health Clinic is one of several community partners identified and authorized during the grant application process. Mendocino Community Health Clinic serves the target population of inland Mendocino County. Under the guidelines specified in this Agreement, the contractor will participate in all relevant activities related to the implementation of the WPC Pilot Program.
