Approve retroactive Agreement with Mendocino Coast Clinics in the amount of $60,000 to provide support, implementation, and participant-specific data to the County of Mendocino’s Whole Person Care Pilot Project for the term of July 1, 2017, through December 31, 2017; authorize the Health and Human Services Agency Director or designee to sign any future amendments to the Agreement that do not increase the annual maximum amount; and authorize Chair to sign same.

Summary of Request:
The Whole Person Care Pilot (WPC) Program is a significant, new, grant-funded project with substantial needs for community participation. The goal of the WPC program is to provide comprehensive coordinated care resulting in better health outcomes among participants with Severe Mental Illness. Mendocino Coast Clinics is one of several community partners identified and authorized during the grant application process. Mendocino Coast Clinics serves the target population of the coastal region of the county. Under the guidelines specified in this Agreement, the contractor will participate in all relevant activities related to the implementation of the WPC Pilot Program.
Funding for this Agreement is provided by the federal Department of Health and Human Services Centers for Medicare and Medicaid Services through the California Department of Health Care Services. The funder notified the County in late June 2017 of the award. Final directions about invoicing and reporting were not issued by the funder until mid-October, and were required to be incorporated into the subcontract language. These delays impacted the County’s ability to complete the contract before the start date of the project.
